Patient Responsibilities: a. Provide accurate and complete medical history information. b. Follow medical advice and treatment plans prescribed by our healthcare professionals. c. Notify the clinic of any changes in contact information promptly.


Appointments: a. Appointments can be scheduled online, by phone, or in person. b. Cancellations must be made at least 24 hours in advance to avoid cancellation fees. c. Late arrivals may result in rescheduling of appointments at the discretion of the healthcare provider.
